PIP Evidence & Real-Life Examples Workbook
Preparing information for Personal Independence Payment can mean trying to remember lots of everyday examples, support needs and changes across better and more difficult days.

This 22-page editable Microsoft Word workbook provides a structured place to record your own experiences across the PIP daily living and mobility activities.

It is designed to help you organise what actually happens in everyday life — using your own words and real-life examples — rather than telling you what you “should” write.

The workbook covers all 12 PIP activity areas: 10 daily living activities and 2 mobility activities, with prompts to consider whether an activity can be completed safely, to an acceptable standard, repeatedly and within a reasonable time.

Each activity page is designed to help you record what happens on different kinds of days, the help or equipment you use, safety issues, whether the activity can be repeated, time taken, real-life examples, after-effects and relevant supporting information.

FLUCTUATING CONDITIONS
A single good or difficult day may not show the whole picture.
The workbook includes space to record better days, typical days, difficult days, changes across the day, effects after an activity, pain, fatigue, distress or other after-effects, and how often difficult days occur.
This helps you keep a more balanced record of how difficulties vary over time.

ORGANISE YOUR SUPPORTING INFORMATION
The Evidence Map gives you one place to note information you already have or may want to refer to, including GP or consultant letters, medication information, treatment records, care or support plans, appointment notes, diaries, symptom trackers, aids and other relevant records.
There is also an Evidence & Example Cross-Reference page so you can link important real-life examples to the information that supports them and record where to find it.

YOUR OWN WORDS — NO POINTS OR SCORING
This workbook deliberately does not calculate PIP points, reproduce descriptor scores, predict whether you will receive PIP, tell you what to say or guarantee an award.
It is an independent preparation and record-keeping resource designed to sit alongside your claim form, diary, appointments, medication information and other evidence.

EDITABLE AND FLEXIBLE
Type directly into the Microsoft Word document or adapt the pages to suit your circumstances.
You can complete the workbook in any order and remove pages that are not relevant to you.
Microsoft Word is recommended for the most consistent formatting.

Frequently Asked Questions

What will I receive?

You will receive the editable A4 Microsoft Word documents listed in the individual product description. They are supplied as an instant digital download. Nothing will be posted to you.

Do I need the newest version of Microsoft Word?

No. The files are supplied in .docx format and should open in versions of Microsoft Word that support this file type. Very old versions may occasionally display small formatting differences.

Can I edit and print the templates?

Yes. You can type directly into the documents, save your completed copies and print them when needed. Please check the page layout before printing, particularly after adding large amounts of text.

Can I use the files in Google Docs, Pages or another program?

The templates are created and checked in Microsoft Word. Other programs may open .docx files, but formatting can change, so compatibility and page layout cannot be guaranteed outside Microsoft Word.

Who are these resources for?

The resources are created primarily for parents and family carers supporting children, young people and adults with learning disabilities. Completed documents may also be shared with relevant health, education and social care professionals.

Are these official NHS or local authority forms?

No. These are independently created, person-centred templates from Learning Disabilities Support Studios. They are not official NHS, local authority, education or social care documents.

Do the templates replace professional advice or existing care plans?

No. The templates help organise and share information, but they do not replace clinical records, professional assessments, prescribed guidance, care plans, risk assessments or emergency advice.

What should I do if I have difficulty opening a file?

Please message me through Etsy and explain what is happening. I will be happy to help you check the download and resolve problems with the supplied files.
