How to use it
Complete the plan gradually with the child or young person wherever possible. They can take part using speech, writing, drawing, symbols, choices, actions, facial expression or another communication method. Begin with the pages that feel most important, then use the one-page summary to share the key information with the adults who support them.

Review the plan when the child says something is not working, when needs change, after a significant transition or at an agreed interval. Keep what helps and change what does not.
