A warm, editable profile with 22 designed A4 content pages to help families understand a child or young person's individual sleep experience and agree practical support for bedtime, settling, night waking, health, recovery and review.
Help the adults around a child understand what they may need at bedtime and overnight - without blame, shame or pressure.
Sleep difficulties are not a sign that a child is naughty, attention-seeking or deliberately refusing to settle. Bedtime and sleep can be affected by anxiety, pain, sensory needs, body rhythms, communication, health, medication, the environment and what has happened during the day. This editable Bedtime and Sleep Support Profile helps families understand the child's individual sleep experience and agree support that feels safe, realistic and person-centred.
The profile keeps the child's communication, preferences and experience central. It helps adults notice patterns, explore possible barriers and agree a predictable but flexible approach to wind-down, settling, night waking and the morning after a difficult night.

How to use it
Complete the profile gradually with the child or young person wherever possible. They can take part using speech, writing, drawing, symbols, choices, actions, facial expression, behaviour or another communication method. Begin with the pages that feel most important, then use the one-page summary to share key information with relevant adults.
Review the profile when the child communicates that something is not working, when needs or medication change, after illness or a significant transition, or at an agreed interval. Keep what helps and change what does not.
Important professional information
This is a blank support-planning and information-sharing template. It does not diagnose a condition, assess a sleep disorder or replace individual medical, educational, therapeutic or social care advice. It does not replace safeguarding procedures, emergency assistance, or an existing care, health, medication, seizure or safety plan.
Seek appropriate professional advice for a new or significant change in sleep, persistent pain or illness, loud snoring or breathing pauses, unusual movements or episodes, extreme daytime sleepiness, medicine concerns, or a risk of harm. In an immediate emergency, follow local emergency procedures.
Compatibility
The file is supplied in .docx format and is intended for Microsoft Word versions that support .docx files. Very old software or other word-processing programs may display small formatting differences, so check the pages after opening. Because the document is editable, page layout may move if large amounts of text are added or fonts are changed.
Digital product
This is an instant digital download. No physical item will be posted. The buyer downloads and edits the file themselves; no Etsy personalisation field is needed.
Personal-use licence
For personal use with one child or young person. The purchaser may print copies and share a completed version with people directly involved in supporting that child. The blank template must not be resold, redistributed, shared with unrelated families, uploaded elsewhere or adapted for commercial use.

Frequently Asked Questions
What will I receive?
You will receive the editable A4 Microsoft Word documents listed in the individual product description. They are supplied as an instant digital download. Nothing will be posted to you.
Do I need the newest version of Microsoft Word?
No. The files are supplied in .docx format and should open in versions of Microsoft Word that support this file type. Very old versions may occasionally display small formatting differences.
Can I edit and print the templates?
Yes. You can type directly into the documents, save your completed copies and print them when needed. Please check the page layout before printing, particularly after adding large amounts of text.
Can I use the files in Google Docs, Pages or another program?
The templates are created and checked in Microsoft Word. Other programs may open .docx files, but formatting can change, so compatibility and page layout cannot be guaranteed outside Microsoft Word.
Who are these resources for?
The resources are created primarily for parents and family carers supporting children, young people and adults with learning disabilities. Completed documents may also be shared with relevant health, education and social care professionals.
Are these official NHS or local authority forms?
No. These are independently created, person-centred templates from Learning Disabilities Support Studios. They are not official NHS, local authority, education or social care documents.
Do the templates replace professional advice or existing care plans?
No. The templates help organise and share information, but they do not replace clinical records, professional assessments, prescribed guidance, care plans, risk assessments or emergency advice.
What should I do if I have difficulty opening a file?
Please message me through Etsy and explain what is happening. I will be happy to help you check the download and resolve problems with the supplied files.
